Initial business application

This free first step records the summary information needed for cohort consideration. It is not the priced Entry Check or an assessment and it does not reserve or grant AIGMS™ Verified status.

Before you apply

Use summary information only

The application should take around ten minutes if you have your Activity Register summary to hand. Do not attach or paste governance evidence.

Information to provide

  • Your legal business name, any trading name and company number where applicable
  • Your principal business address
  • Your website and business sector
  • The function, service or location you want examined
  • The number of Active AI Activities in that scope
  • The highest Consequence Rating present
  • The AIGMS™ version in use and date it began operating in the proposed scope
  • The start and end dates of the operating evidence period
  • Whether an Assurance Review has been completed
  • Whether external implementation support was used

Information not to provide

  • Your Activity Register or Governance Evidence Pack
  • Employee, customer or service-user personal information
  • Incident, complaint, legal or regulatory records
  • Health information or other sensitive information
  • Passwords, access credentials or confidential evidence links
  • AI provider data, outputs or commercial material
No evidence upload is required.

If the summary information appears consistent with the published eligibility conditions, the Programme may consider the business for the controlled cohort. The current intake stage does not authorise payment, an Entry Check, evidence access or assessment. Any later pilot activity would require separate recorded approval and written participant terms before evidence arrangements are discussed.

All required fields relate to your organisation and readiness summary. The form must not be used to transmit governance evidence or sensitive information.

Full name
Name of the authorised business representative.
Your role in the applicant business.
The legal entity making this application.
State any trading name and company number. If neither applies, enter Not applicable.
Enter the current total recorded in the Activity Register.
Use day, month and year.
Highest Consequence Rating in the proposed scope
Use the highest rating recorded for any in-scope AI Activity.
Identify the functions, services and locations you want the cohort work to cover. Do not paste evidence.
Use an address belonging to the applicant business where possible.
Has an Assurance Review been completed?
At least one complete Assurance Review is required before cohort assessment can begin.
State the AIGMS™ version in use plus the evidence start and end dates.
Do you have at least 90 consecutive calendar days of genuine operating evidence?
The evidence must have been created while AIGMS™ was operating.
Was external implementation support used?
Include consultancy, training or organisation-specific implementation support.
Provide the applicant business's principal geographical address.
Provide the website address and a short description of the business sector.
If support was used, identify the provider and briefly describe the relationship. Do not include contracts or confidential material.
